TIME REQUIREMENT

• Payable for death of an insured contributor, caused by employment injury

• Payable to spouse, if no spouse then the person paying the funeral expenses

• Present death certificate of deceased insured contributor

• Present written approval from Social Security accepting liability for employment injury. 

• If no spouse, the person who paid the funeral expense must produce receipts.

• Make claim on prescribed form (FG2) signed and dated by the claimant

• If claim is made more than one year after death, no sum shall be paid
