Dear valued pensioner,
To ensure that you continue to recieve your pension monthly, please be sure to fill out a life declaration form and hand it in to the Social Security Board every six months. We ask that you fill it out during June and December yearly, and the form will be valid for the proceeding six months.
Please fill out form P6 RB/DIS/INV if you are receiving a Retirement Pension OR a Disability Pension OR an Invalidity Pension.
If you are receiving a Survivor's Pension or Death Pension, please fill out this form P6 SVB/DB. If you have children who are 16 years and not yet 21 years of age, proof of education must be submitted.
- Submit proof of education for children who are attending high school in September of each year.
- For children attending sixth form or university, proof of education must be submitted in January and September of each year.
If you are receiving Non-Contributory Pension (NCP), kindly fill out form P6 NCP. This form must be filled out by the pensioner and not the person who is authorized by the pensioner to collect.
