HOW TO QUALIFY

YOU MUSTREQUIREMENTSTIME REQUIREMENT
The deceased insured contributor must have no less than 50 contributions since 1981Present original death certificate Make claim on prescribed form (FG2) signed and dated by claimant
For death of an insured contributor’s dependent (spouse or child), the insured contributor must have no less than 150 contributionsProof of payment of funeral cost by way of original receipts in the name of the applicant or proof of liability to pay by way of original bills or pro forma invoice in the name of the applicant.

The dependent child of the deceased insured contributor must be unmarried and under the age of 16 years, or 21 years if receiving full-time education at the time of deathClaim should be made within 6 months from the date of death of the person
The spouse can be either by marriage or proven common-lawIf claim is made more than one year after death, no sum shall be paid

RATE & PAYMENT OF BENEFIT

RATES OF BENEFIT PAYMENTPERIOD OF BENEFIT PAYMENT
$1500 for the person bearing the funeral expenses of the deceased insured contributorOne lump sum payment
$1,000 is paid in the case of a deceased spouse
$500 is paid in the case of a deceased dependent child of the insured contributor
