Short Term: Maternity Grant
MATERNITY GRANT (One Lump Sum)
Maternity Grant is a lump sum payment of $300.00 made upon confinement. Maternity Grant may be paid to an Insured Man if his wife does not qualify for the benefit.
Maternity Grant of $300.00 is payable per child in one calendar year.
HOW TO QUALIFY
|Be an insured woman||The insured woman must have claimed Maternity Benefit on the prescribed form (MB1)|
|Have been confined||A male claimant (insured man) must make claim on the prescribed form (MB5)|
|Have 50 paid contributions||MB5 must be fully completed and signed by the insured contributor|
|Have 25 paid contributions in the 50 weeks before confinement||Must present Birth Certificate or certificate of registration of birth of child, showing claimant as the father|
|Or | Be an insured man||Employer must declare claimant’s employment status. Record the salary or wages (SM2/MB form)of the insured man 50 weeks before confinement.|
|Have 50 paid contributions||Submit claim within 3 months after date of confinement|
|Have 25 paid contributions in the 50 weeks before spouse’s confinement||If claim is not submitted within 3 months after date of birth, a good reason must be given by the insured person and recorded on the MB5.|
|WIFE, (married or common-law) has been confined|
|WIFE does not qualify for the benefit|
